Pivotal role for proteins: From helping turn carbs into energy to causing devastating disease

ScienceDaily (May 24, 2012) ? Research into how carbohydrates are converted into energy has led to a surprising discovery with implications for the treatment of a perplexing and potentially fatal neuromuscular disorder and possibly even cancer and heart disease.

Until this study, the cause of this neuromuscular disorder was unknown. But after obtaining DNA from three families with members who have the disorder, a team led by University of Utah scientists Jared Rutter, Ph.D., associate professor of biochemistry and Carl Thummel, Ph.D., professor of human genetics, sequenced two genes and identified two mutations that cause this devastating disease.

"The ability to convert carbohydrates into energy is critical for people and other organisms to live. But when that process goes awry, potentially fatal health problems can occur," Rutter says. "If we can figure out a way to correct the defects, we might be able to treat the disease."

Rutter and Thummel are senior authors on a study published online in Science Express on May 24, 2012.

The researchers studied two proteins, Mpc1 and Mpc2, which are among a dozen proteins they looked at in fruit flies, yeast, and then humans. They discovered that the two proteins play a pivotal role in the cellular process that produces the majority of ATP, a molecule that is the main source of energy for cells and is essential for people and other animals to live. Rutter and his colleagues also discovered that when Mpc1 and Mpc2 are impaired they cause the deadly and as of yet unnamed neuromuscular disorder. This disorder affects thousands of people worldwide.

To produce ATP, the body metabolizes carbohydrates and converts them into pyruvate, which then typically enters into the mitochondria in cells. Once inside the mitochondria -- a self-contained unit often referred to as a cellular power plant -- pyruvate is consumed in the production of ATP. Rutter and his fellow researchers discovered that Mpc1 and Mpc2 are critical for pyruvate entry into mitochondria. When Mpc1 and Mpc2 are eliminated or mutated, pyruvate cannot enter into mitochondria and ATP is not efficiently produced -- and that's when serious health problems can arise, including the neuromuscular disorder that in its most severe forms is deadly.

The ramifications of this study go beyond the production of ATP and birth defects seen in the neuromuscular disorder. The findings may be useful in understanding some of the metabolic defects associated with cancer and heart disease, according to Rutter. Cancer cells typically don't consume their pyruvate in the production of ATP at the same rate as normal cells. Instead, they convert the pyruvate to lactate. This property of cancer cells is called the Warburg Effect and is named after Nobel laureate and cancer researcher Otto Heinrich Warburg. Some forms of heart disease have a similar problem.

Further study based on the current research may provide important information regarding those diseases, according to Rutter. "That might be the most important outcome of our studies in the long run," he says.

The study's first author is Daniel K. Bricker, a doctoral student in human genetics at the University of Utah. Researchers from Harvard University, and the Laboratoire de Biochimie and the Institut de Genetique et de Biologie Moleculaire et Cellulaire, both in France, also contributed to the study.

    ST. CATHARINES ? The Evolution of Internal Dialogue exhibition featured artwork produced by Laura Secord visual arts students in St. Catharines from Grade 9 to Grade 12.

    This exhibition was organized and curated by the senior DNA visual arts students under the supervision of Mr. S. Azzopardi and Mr. S. Simpson and showcased work done in a variety of media including drawings, paintings, installations, sculptures and photo manipulations.

    The end result was a gymnasium full of thought provoking art that demonstrated the result of the internal dialogue that occurred during the creative process for each of the participants.

    All aspects of popular culture seemed to be touched on in the exhibit that featured lots of variety. From a black lit Starbucks poster, to sinister drawings of faces manipulated with fangs and horns to the more cheerful large oil and canvases featuring a flower, it was there.

    Azzopardi says it is vital to bring the art into the school system for the kids.

    ?It?s really important because it is a skill set that we believe the students really need to practice and to hone so that when they go to post-secondary education they have that.?

    He says what they try to do at the school is to go for a nice balance between the kid?s technical ability which is how well they can draw and mix colours, to creativity which includes being able to put that creative spin on something and putting creative concepts together. He explained further the title of the show.

    ?The evolution part is the student?s growth throughout their years until Grade 12 and by Grade 12 they develop their own style.?

    Once this level has been established, the kids create what is called their masterpieces which are their signature pieces for the show. Thus the entire journey is an evolution.

    A big part of the creation of the art is troubleshooting as well he says and learning how to express a concept that you are trying to convey. The good news is that this skill set becomes very transferable to other occupations that the kids can use in their future careers. From interior design to illustrating careers to you name it.

    Azzopardi stressed that the show was also about letting the kids flex their creative muscles as well as providing a platform to showcase the artwork they have worked so hard on all year.

    He says ?it?s great to make it but then when you actually have it set up for public viewing, how are you reacting to that work and how is the public reacting to that work are great indicators for the students so they can learn.?

    Andrew Shaefer, student, exhibitionist and one of the co- head curators for the show talked about the hard work that went on behind the scenes to pull the whole thing together.

    ?It took a lot of planning. A lot of running around between different teachers making sure we had the gym ready to go, that all the students had their artwork ready to go. Trying to format all the little labels and the artist statements, trying to compile all the artwork and sort through it so that we could figure out what we did want and what we didn?t want. We wanted to make sure we were featuring lots of variety and lots of the best artwork that we could.?

    Shaefer said he thought the show was amazing because often kids in the art programs get a little left out at school functions.

    ?A lot of the time we have music, we have concerts, we have sports, we have musicals and plays for the drama people, but the art kids kind of get left out sometimes so it?s good to have a nice big show where we get lots of people out where we can exhibit all the work and really show the talent that?s in our school.?

    ATS 2012, SAN FRANCISCO A tiny, resilient metal wire designed to gather and compress diseased lung tissue may offer relief to patients with severe heterogeneous emphysema, a subtype of the disease that involves specific, usually isolated areas of the lungs, according to the results of a multicenter international trial conducted in the Netherlands, Germany and France. The wire, called a lung volume reduction coil (LVRC), can be easily implanted and is designed to take the place of more invasive procedures used to improve the lung function of emphysema patients.

    The study will be presented at the ATS 2012 International Conference in San Francisco.

    "Our results indicate that LVRC treatment is technically feasible in both upper and lower regions of the lung in patients with severe heterogeneous emphysema, and results in significant improvements in pulmonary function, exercise capacity and quality of life, with an acceptable safety profile," said study lead author Dirk-Jan Slebos, MD, pulmonary physician at the University Medical Center Groningen (UMCG), The Netherlands.

    Lung volume reduction is performed in patients with emphysema in an effort to isolate diseased lung tissue and allow healthy lung tissue to expand and function more efficiently, which in turn helps the patient to breathe and function better. Because heterogeneous emphysema involves different areas, or lobes, of the lung, surgery to contain diseased lung tissue can be complicated and extensive. Use of wire coils, which can be implanted in relatively noninvasive bronchoscopic procedures, is emerging as a more effective, and safer, option than surgery for these patients. Coils are compressed during implantation, and expand to their original shape once implanted in the lung tissue.

    Although a previous single-center trial showed LVRC to be safe and effective in patients with severe upper-lobe emphysema, Dr. Slebos said this study involved patients treated at 11 different healthcare centers, and explored the safety, efficacy and feasibility of LVRC in both upper and lower lobes.

    The study enrolled 53 patients who underwent 101 LVRC procedures, including 46 bilateral (both lungs) procedures and nine unilateral procedures. In each procedure, the clinicians implanted from eight to 14 coils, resulting in a study total of 1,070 coils being implanted. Safety was evaluated by recording all adverse events that occurred within the first 30 days following treatment, as well as those that occurred between 30 days and six months of treatment. Efficacy was measured by quality-of-life questionnaires, pulmonary function testing and exercise testing, all performed six months following treatment. Reported adverse events included exacerbation of chronic obstructive pulmonary disease (COPD), pneumonia, collapsed lung, chest pain and mild hemoptysis, or coughing up blood, which could be managed with standard care.

    The researchers found that LVRC treatment resulted in significant improvement in respiratory measurements at six months, especially in patients whose lungs were severely hyperinflated. In this group, LVRC therapy resulted in significant improvements in lung function, hyperinflation, exercise capacity and quality of life measurements. In fact, severe hyperinflation at baseline appeared to be an indicator of better outcomes, Dr. Slebos noted.

    "Hyperinflation of lung tissue is one of the key pathophysiological features of emphysema that is responsible for shortness of breath, and its magnitude seems to be a good indicator of successful LVRC treatment," he said.

    Dr. Slebos said the trial is ongoing and continues to collect data.

    "The results so far indicate that the LVRC is a safe and effective choice for treating patients with severe heterogeneous emphysema where current medical treatment fails, and as we continue to collect follow-up data, we hopefully will have a more complete profile of its applications in this patient population," he said.

Safety was evaluated by recording all adverse events (AEs), efficacy was measured by questionnaires, pulmonary function and exercise testing at 6 months post treatment. The trial is ongoing.

    Results: 101 LVRC procedures were performed (46 bilateral, 9 unilateral). 40 patients were treated in the upper lobes and 13 in the lower lobes. A total of 1070 coils were placed with median 10 coils (range 8-14) placed per procedure in 4824 min. Adverse events 30 days 6months after the procedure were: COPD exacerbation (n=21), pneumonia (n=11), pneumothorax (n=3), chest pain (n=3), and mild hemoptysis (n=1). LVR-coil treatment resulted at 6 months in the overall group in a ?FEV1 +13% (4.9%), ?RV -0.44L (0.16), ?6MWD +35m (12) and ?SGRQ -10 points (2.3). RV%pred at baseline correlated significantly to improvement in RV after treatment (r2=0.453, p220%pred, the 6 months results are: ?FEV1 +19% (6.8%), ?RV -0.57L (0.21), ?6MWD +42m (14), and ?SGRQ -14 points (3.1).

    Conclusion: LVRC treatment is technically feasible in both upper and lower lobe severe heterogeneous emphysema and results in significant improvements in pulmonary function, exercise capacity and quality of life, with an acceptable safety profile. High RV at baseline seems to be an indicator of better outcome.
